New Patient Registration
When you first register you will be asked to make an appointment for each member of your family with our healthcare assistant which will include a simple health check and give us an opportunity to take some brief details of your medical history.
You can arrange this appointment either in person at the time of registering or by telephoning the Surgery on 01769 573101. Please tell the receptionist that you need a “New Patient Check” so that you are given a double appointment which allows us sufficient time to get to know you. Please also bring a urine sample (bottles are available from the Reception Desk).
If you urgently need any medication or have severe ongoing medical problems you will need to make a 20 minute appointment with your registered GP.
It is IMPORTANT that you try to see us as soon as you can after registering so that we can meet you and take your medical history whilst awaiting the arrival of your notes from your previous practice.
We firmly believe in preventing disease and promoting health. For this reason we often ask about lifestyle factors such as diet, exercise and smoking, as well as dealing with whatever problems you have come about. We are willing to help you lose weight, stop smoking and advise about exercise, so please ask for details.
