If you are planning foreign travel, firstly you need to complete a travel questionnaire so that we can ascertain which countries and areas within countries that you are visiting to determine what vaccinations are required. Please complete the one below or ask for one at Reception if you prefer. If you require any vaccinations relating to your foreign travel we will let you know what are required and the length of appointment you need to book. This may also include a discussion regarding any further travel arrangements required such as malaria protection. It is important to complete and submit the questionnaire as early as possible - at least 6 weeks before you travel - as an appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines may have to be ordered as not all are stocked vaccine. The appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Anti malarials can be purchased from the Exeter Travel Clinic without the need for a prescription. However, patients will need to complete a patient assessment and have a consultation with the Travel Health Consultancy so an appointment will need to be booked in advance of attending.
